Manifestations of Dementia Melinda Godfrey GNUR543 St. John Fisher College Mrs. Yowell is a 90-year-old woman who is a resident of a long-term care facility. She was alert and mentally quite capable until about a year ago when she began to manifest signs and symptoms of dementia. A review of her medical records failed to document a thorough analysis of her dementia, but a diagnosis of â€Å"probable Alzheimer disease† was recorded. What are the common manifestations of dementia? The definition of Dementia is â€Å"a general term for loss of memory and other mental abilities severe enough to interfere with daily life.It is caused by physical changes in the brain. † (Common Types of Dementia, 2012). The first manifestations of Dementia usually are: * Loss of memory – generally the patient doesn’t notice the loss of memory but a loved one will. This is generally the entree into the physician’s office leading to a diagnosis. * Trouble focusing and follo wing conversations – the patient is unable to handle more than one task at a time and will not be able to perform a task and listen to or follow a conversation easily. * Impaired judgment and reasoning – patients become confused and are unable to cope as well when unexpected events come up.Other symptoms could include: mood changes, personality and behavior changes. As noted, dementia is not a disease itself but a cluster of cognitive changes. The onset symptom of dementia can vary depending on the disease or syndrome that is associated (Common Types of Dementia, 2012). The major dementia diseases or syndromes are: 1. Alzheimer Disease (AD) – AD accounts for 60 – 80% of the cases of dementia (Shadlen & Larson, 2012). 2. Dementia with Lewy Bodies (DLB) – DLB starts with progressive cognitive decline and usually the patient also has hallucinations, periods of lucidity and some rigidity.DLB has abnormal accumulations of protein structures in the patie nt’s brain (Dementia With Lewy Bodies Information Page, 2011). 3. Frontotemporal Dementia (FT) – FT is associated with the shrinking of the frontal and temporal anterior lobes of the brain (Frontotemporal Dementia Information Page, 2011). Generally the patient will have trouble with speech and behavior. 4. Vascular Dementia – This form of dementia results from many strokes. When a patient has a stroke, the blood flow is interrupted to the brain and the result is brain tissue damage (Types of Dementia, 2011).These patients normally lose their cognitive ability before their memory. 5. Parkinson Disease with Dementia – Approximately 40% of patients with Parkinson’s disease will have dementia (Parkinson’s disease, 2011). The nerve cells in the brain that make dopamine are slowly destroyed leaving no way for the brain to send messages. What other potentially treatable factors might have led to Mrs. Yowell’s deteriorating mental function? T here are many reasons that Mrs. Yowell may have impaired mental functioning (see Table 1). The first and easiest reason to rule out would be a urinary tract infection (UTI).A urine sample can show if there are white blood cells (indicating infection) present in the urine. The increased levels of bacteria in an elderly person’s body can cause toxicity which leads to the altered mental status (Midthun, 2004). Most noted changes are confusion, agitation and lethargy. Table 1| | (Shadlen & Larson, 2012)| Secondly, Mrs. Yowell could have vitamin deficiencies. For example, if a patient has low levels of thiamine they can develop Wernicke’s encephalopathy. This can cause damage in the brain to the thalamus and the hypothalamus.These patients will exhibit signs of confusion, loss of memory and hallucinations (Dugdale, 2010). If left untreated it can cause permanent damage but, it is usually corrected by injections of thiamine. Another cause of confusion/delirium in elderly pat ients is an adverse drug reaction or interactions with another drug. Mrs. Yowell could have been given a new medication that is directly affecting her mental status. Nearly 70% of residents in long term facilities take nine or more medications (Gillick, 2012). Elderly patients have slower clearance rates of drugs through their liver and kidney thus leading to drug accumulation (Gillick, 2012).There should be a regular review of medications in order to anticipate and keep on top of any potential changes. Discuss in detail the organic brain/neuronal alterations that are typical of Alzheimer disease? The changes that occur in the brain with Alzheimer’s disease (AD) are threefold. The first is amyloid plaque, this is abnormal clumps of protein found outside of the nerve cells. The protein pieces are in the fatty membrane that is around the brains nerve cells. The amyloid is a protein that is snipped from a larger protein (amyloid precursor protein) during metabolism (Copstead, 20 10).These clumps form plaque and can block cell-to-cell signaling (Common Types of Dementia, 2012). In a healthy brain the fragments are broken down and eliminated before they turn into plaque. Without the ability to signal the brain may also activate the inflammatory response. At this point it is still not known whether the plaque causes or is created by AD. Secondly, inside the nerve cells there is a protein called tau. This protein is normally helps maintain the cell structure allowing proper nutrition and signaling. The tau protein in AD is changed into twisted strands, called tangles (Common Types of Dementia, 2012).It is said that the changes are brought on by the phosphorylating enzymes which are activated by inflammatory changes, lipid abnormalities and aging (Copstead, 2010). Thirdly, the brain is losing connectivity between nerve cells and cells are dying which results in permanent brain damage. Other changes in the brain include shrinking of the hippocampus and cortex. AD also shows an increase in ventricles. The progression of the damage in the AD brain is predictable. According to the Alzheimer’s research center, the plaques and tangles spread through the cortex in a predictable fashion (Common Types of Dementia, 2012).Summary Scientists believe that the cause of AD is multi-factorial. IT could be a combination of genetics, lifestyle and environmental factors. Specifically, it is believed that a person with diabetes, depression or heart disease is more likely to develop AD (Copstead, 2010). There is a link to a gene, APOe4 that is thought to be associated with late onset AD. However, studies have shown it is not present in some cases of AD and is present in cases with no AD (Common Types of Dementia, 2012). At this point the definitive diagnosis of AD can only be obtained after death (Common Types of Dementia, 012). This means that clinicians must ask the right questions to diagnose this disease. AD is still quite unknown as to the order of the changes that occur in the brain. The popularity of fast food restaurants is growing because of three main causes: the low cost, the fast service, and how fast food restaurants are available anywhere. The most evident advantage of fast food is than it saves time. In today’s fast-paced life, there is nothing better than getting a ready meal. No matter how much the chefs praise the benefits of fresh food, at the end of a hard-working day, when one returns home all tiered and hungry? A pizza or a burger can be godsend. Besides the time an individual has to spend in the kitchen, cooking a meal also requires one to make a trip to the supermarket to buy the ingredients for the dish. Then there is the added effort and time consumed in washing and reeling the vegetables. All this makes eating fast food score more preferably over cooking a meal for a busy individual. Besides time, cost saving gives fast food an edge over the meal prepared in the kitchen. If one lives alone, then it is cheaper to buy or order a meal in the fast foods’ restaurant instead of cooking it at home. Also certain fast foods like fries and burgers come pretty cheap. Fast food does raised health concerns. However if careful, you can find some options on the menu of fast food restaurant that could be healthier. Salads are a smart choice. Go for bread products that are made from wheat bread. Opt for lean meat. If you could choose between the fried and boiled options, order the boiled preparation. Avoid ordering carbonic drinks, when you are thirsty. Go for fruit juices, low-fat milk, diet soda. It there anything better than plain water to quench your always resort to the â€Å" make to order† option that certain fast food outlets offer, where you could restrict the use of ingredients that are not healthy. As dessert does not play on important role in the fast food meal menu, you can keep yourself away from delicious and tempting sugary dishes.

There are, therefore, no restrictions on the movement of labor from one firm to another and there is no trade union either. In addition, no firm can control industrial input; hence, there is perfect mobility of capital as well. Another concept common to perfect competition is the free entry and exit of firms in the industry. This sh eds light to the fact that there are no legal, financial or market barrier for any firm to enter or exit the industry. Firms can choose to enter or exit at their free will. When the industry is enjoying abnormal profits, that is when the short run average cost is less than the price, and then firms enter the industry. However, when the abnormal profits are transferred into normal profits or losses, then firms leave the industry (Dwivedi, pp. 297, 298). This model makes a further assumption that there is ‘perfect knowledge’. This suggests that there is no uncertainty in the market, and information regarding the market is readily available and is free of cost. In addition, firms act independently and they do not collude with each other in any way. Furthermore, there is no government intervention in perfect competition. There are no discriminatory taxes or subsidies, government does not put up a maximum or minimum price and does not have any sort of direct or indirect cont rol. Such characteristics make this model unique (Dwivedi, pp. 297). â€Å"The demand curve of a perfectly competitive firm is horizontal; this signifies that the firm can sell as much as it wants at the prevailing market price† (Dwivedi, pp. 298-300). Any firm in perfect competition is so insignificant that it absolutely has no influence over price. The diagram is shown below. (McEachern, pp. 23-25) This characteristic of perfect competition also makes it unique in all types of market structures. In addition to that, perfect competition is used as a ‘useful benchmark’ to judge the efficiency of markets. There are two broad concepts of efficiency, allocative efficiency and productive efficiency. Productive efficiency occurs when the firm is producing at the minimum of its long run average cost curve (LRAC). This signifies that the cost is less than the market price of a certain product. In perfect competition, output is produced at the minimum of average cost in the long run. Generally the sentimental plot is written to make the reader feel an excess of unnecessary emotions, by overemphasizing every work. In every sentimental plot there will always be the typical sentimental hereon that fits the stereotype of a young orphan, unmarried women with moral obstacles. In this plot there are only two types of women the ones that are considered good and the ones that are bad. In the sentimental plot the good will get rewarded by marrying a decent man and the bad will always die. Even though this plot was very popular among writers many women were not happy with how the women were being portrayed among men writers so they decided to go against this plot. With women writers some would tend to go against the sentimental romance plot and instead write about actual situations that were happening around them. One example of this would be in Kate Chopin’s story â€Å"The Storm† it featured a married women falling into the seductions of a old flame. Do you remember in Assumption, Calixta? He asked in a low voice broken by passion. Oh! She remembered; for in Assumption he had kissed her and kissed and kissed her; until his senses would well nigh fail, and to save her he would resort to a desperate flight. If she was not an immaculate dove in those days, she was still inviolate; a passionate creature whose very defenselessness had made her defense, against which his honor forbade him to prevail. † Pg. 59 Author Kate Chopin was unable to publish her works because of the featured adultery with no follow up punishment, in order for her to successfully publish she would have to follow the sentimental romance plot and kill Calixta in the end to prove that the decisions she made were wrong and had consequences. Mary E. Wilkins in â€Å"The Revolt of Mother† is another example that goes against the grain of the sentimental plot it deals with a mother that confronts her husband and goes against his say so which in the 19th century was something that was frowned upon. Wilkins explained that no New  England women would ever dare confront her husband it was all just a fantasy. Another example of the reversed sentimental plot would be in Charlotte Perkins Gilman story â€Å"The Yellow Wall-paper† It is based on a women who is said to be having nervous breakdowns and is being taken care of by her husband a physician she later comes to relies that she will no longer be trapped or hide her emotions. â€Å"I’ve got out at last. Said I, in spite of you and Jane! And I’ve pulled off most off the paper, so you can’t put me back! † pg 803. What all of these story’s have in common is that they tried to convey across the situations that the women were in and all of them one way or another expressed how the women felt trapped by the men. When it came to men writers they would always include a sentimental hereon in their stories. Henry James published â€Å"Daisy Miller† in 1978 and it dealt with a young unmarried woman somewhat considered an orphan because her parents were never around, who was seen in society as an outcast cause of the way she would conduct herself around men. I went to Juab High School in the small town of Nephi, Utah. Like many other small town high schools, football coaches and P.E. teachers doubled as Algebra teachers and Science teachers. This allowed our school to make full use of the limited teachers and resources that it had. There was a lot of talented people that taught at Juab and some of them made great teachers and coaches, but some of them didn't. Sometimes it ended up that the football coach/algebra teacher cared a little more about tomorrow's football game than he did about ensuring his algebra students knew how to balance equations, and sometimes the P.E./Science teacher cared a little more about the teaching the tennis unit than she did about teaching the four life processes. Those teachers were also the ones that had to relearn the algebra and science lessons a few days before they taught them to us, because on paper they were qualified to do the job, but as far as knowing the material and having an interest in what they were trying to give to us, nothing was there. Have you ever tried to get someone excited about a subject that you knew nothing about? Have you ever had a math teacher that sent you across the hall to get help from someone else because he didn't understand what he was trying to teach you? It can be pretty hard sometimes to get excited about something if your teacher doesn't get excited about it. These teachers tried to give us something that they didn't have. When I was in middle school I had another teacher that tried to give us what she didn't have. She was the health teacher, but because of some addictions to drugs, she really wasn't very healthy. It was sad, because she taught us from the book t... ...t a fantastic one." If you were to give someone a gift and then watched them neglect it, abuse it, or destroy it, your feelings would be hurt and you would be pretty upset. But if you were able to see them show it off to their friends, love it, and take care of it, you would feel like that person appreciated the gift and it would be almost like a gift to you. Upon reading this quote is where I took the opportunity to look into the future. The gift that God gave me is a wonderful one. Sometimes I have a hard time seeing my life as being wonderful because I make so many mistakes. But life is wonderful because life is about making mistakes. It is what we do with them that is important. I know that God always sees me as being wonderful. God loves me. He gave me life. God wants me, and every other person, to become the very best that we can be, and then to become even better. He wants us to nurture life, to love life, and to live it. Let's do our best.
